New Patient Registration

We accept new patients who live within the practice area. The surgery boundary stretches from Shaftsbury Square to the start of Lisburn reaching out to either side of the Lisburn Rd in a long rectangle.  Please contact reception for further information.

Please contact reception to arrange collection of new patient registration forms. You will also need to provide some form of photo ID and a utility bill with your current address

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice. You must bring completed forms and ID to reception to get registered.

IF THIS IS THE FIRST TIME REGISTEREING WITH A GP IN THE UK AND YOU HAVE MOVING FROM ANY EU COUNTRY YOU MUST COMPLETE THE FORM ON THE FOLLOWING LINK - https://www.gov.uk/settled-status-eu-citizens-families. Your registration can not be processed in the surgery without this.

Changes to Personal Details

If you change your name, address or your telephone number please let the surgery have your new details.
